A review published in Frontiers in Nutrition consolidates ten peer-reviewed clinical studies on the probiotic strain Lactiplantibacillus plantarum 299v (Lp299v, marketed as LP299V by Probi AB) and its relationship to non-heme iron bioavailability.1 For manufacturers formulating iron products or exploring microbiome-informed ingredients, it offers a synthesis of the mechanistic and clinical evidence.
How Big Is the Iron Deficiency Problem?
Iron deficiency is described in the review as the most prevalent micronutrient deficiency worldwide, affecting an estimated 2 billion people and the leading cause of anemia. Non-heme iron, the predominant dietary form globally, is absorbed at rates below 15% and is limited by inhibitors such as phytates and polyphenols. Oral iron supplementation, though a widely-used practice, can cause gastrointestinal side effects in up to 60% of users, undermining adherence.
What Mechanisms Does the Review Describe?
Some of the proposed mechanisms include organic acid production that lowers luminal pH, upregulation of DCYTB, phytate mitigation, and intestinal barrier function.
“Iron status depends not only on how much iron is consumed, but also on how effectively it is absorbed,” explained Brandy Webb, N.D., scientific affairs manager at Probi in a September press release.2 “The research reviewed provides insights into how LP299V may support this process through several complementary mechanisms. Importantly, the evidence positions LP299V as a bioavailability enhancer that can complement dietary iron and iron supplementation rather than replace them.”
What Do the Clinical Findings Show?
Human isotope studies cited in the review report relative increases in fractional non-heme iron absorption of roughly 20% to 50% with LP299V across varied dietary contexts, though one trial using lyophilized LP299V did not show a significant benefit.
Controlled trials show improved iron status across populations: pregnant women had higher ferritin, hemoglobin, and lower iron deficiency rates (59% vs. 78%) versus placebo; adults with iron deficiency anemia saw greater gains and less GI intolerance (13.0% vs. 46.5%); and results in female athletes showed an increase in vigor.
The authors conclude that LP299V functions as a bioavailability enhancer rather than a substitute for iron intake. “Given its versatility in delivery formats, favorable safety profile, and applicability across a range of dietary patterns and socioeconomic settings, Lp299v represents a microbiome-informed approach to addressing the persistent global burden of iron deficiency that has great potential for widespread adoption,” the authors stated.
What Does Regional Research in Japan Show?
Beyond iron absorption, the effects of the strain have been studied in specific regional populations. Earlier this year, Probi and Nomura Dairy reported what they described as the
