Berberine has become an ingredient of interest in the supplement aisle, driven in large part by social media attention to its purported metabolic benefits. But interest may have outpaced the evidence: while some research points to modest effects on blood glucose, cholesterol, and insulin resistance, findings on weight loss remain inconsistent, and much of the underlying research has limitations in size, quality, and geographic scope. Meanwhile, formulators are working to address berberine's bioavailability challenges, and recent product testing has raised questions about quality control across the category.

What Is Berberine?
According to the National Center for Complementary and Integrative Health (NCCIH), berberine is a compound found in several plants, including barberry, goldenseal, goldthread, Oregon grape, phellodendron, and tree turmeric.1 It has a long history of use in Ayurvedic and traditional Chinese medicine for infections, wound healing, and gastrointestinal complaints, and today it's marketed as a dietary supplement for cholesterol, blood pressure, and, most recently, weight management. Ingredient suppliers note berberine's interaction with enzymes that help regulate energy production, glucose metabolism, and immune function.2
Does Berberine Support Weight Loss?
The evidence is mixed. A 2022 review cited by the NCCIH found that 18 studies on body weight and 23 on BMI showed significant reductions with berberine, but effects were seen mainly at doses above 1 gram per day taken for more than 8 weeks, and many of the underlying studies carried a high risk of bias and demonstrated inconsistent outcomes.3 NCCIH notes that berberine's recent popularity on social media has outpaced the clinical evidence as of 2023, and most trials have been conducted in Asian populations, limiting generalizability.
What About Blood Sugar and Cholesterol?
NCCIH's diabetes supplement fact sheet cites a 2021 review of 46 studies (4,158 participants) showing berberine may lower blood glucose, reduce insulin resistance, and improve lipid metabolism in people with type 2 diabetes, particularly as an adjunct therapy, though most studies were conducted in China and quality varied.4 Separately, NCCIH notes berberine may modestly lower blood glucose and cholesterol, and may enhance the blood-pressure-lowering effects of amlodipine.
What Are the Formulation Challenges?
Poor oral bioavailability is a significant challenge. One study cited it at around 2%, which historically has required high divided doses (500–1,500 mg/day) and has been linked to gastrointestinal side effects that hurt compliance.5 This has driven a wave of bioavailability-enhancement approaches: Indena's Phytosome-based Berbevis uses lecithin-based delivery to address absorption6; Gencor's BioBerb applies Pharmako Biotechnologies' LipiSperse dispersion technology for the same purpose2; and Zeus Hygia's Metaberine uses proprietary "BioSOLVE" particle-engineering technology, which the company says enabled a 36-subject crossover trial to show glucose, GLP-1, and lipid benefits at just 200 mg/day over 14 days.5,7 This study's authors caution that findings are exploratory given the small sample, single-center design, and short duration.
Is Berberine Safe?
NCCIH notes that berberine is generally well tolerated when taken orally, though it is not without risks. At doses used in clinical studies (200–1,000 mg two to three times daily), it's considered generally safe, with the most common side effects being mild-to-moderate nausea, diarrhea, bloating, and constipation. Berberine may interact with medications, including cyclosporine, and the NCCIH states that it should not be used during pregnancy, while breastfeeding, or in infants, due to a risk of bilirubin buildup linked to brain damage.
Additionally, because goldenseal delivers little absorbable berberine when taken orally, research on isolated berberine may not translate to goldenseal products, and goldenseal itself carries drug-interaction concerns, including a documented interaction that lowered metformin levels by about 25%.8
Are There Any Quality Concerns in the Market?
In testing of 30 berberine products purchased from Amazon and Walmart.com in 2024, NOW Health Group found every brand tested below 100% of labeled potency except its own; 18 of 33 products tested below 40% of label claims, and seven contained 1% or less of the labeled amount.9
For manufacturers, this underscores the importance of third-party verified potency testing, particularly as consumer demand may continue to outpace quality controls in the broader market.
