Independent Testing Determines Nutrient Levels in Most Prenatal Multivitamins Differ From Labeled Amounts

New testing of popular prenatal multivitamins from Consumer Reports found nutrient levels outside of the voluntary potency standards.

Consumer Reports' testing of 17 popular prenatal multivitamins, published this month, found that only 2 products fell within US Pharmacopeia (USP) tolerance ranges for every nutrient measured.¹

Across the sample, 32% of the 126 nutrient measurements taken fell outside those thresholds, with folic acid, a nutrient tied to prevention of neural tube defects, under-labeled in 5 of the 17 products.

For manufacturers already navigating tightening scrutiny of quality systems, the findings arrive as another data point building the case for verified potency testing rather than voluntary compliance.

What Did the Testing Find?

Consumer Reports purchased 1 to 3 lots of each product and analyzed 9 nutrients, including calcium, iron, zinc, and vitamins A, C, D, and E, alongside 4 heavy metals. Gummy formulations showed the widest swings: half of the 8 gummy products tested contained less folate than labeled, and 1 product reportedly delivered just 28% of its stated folic acid content.

Several products, mostly gummies, exceeded tolerable upper intake levels for certain nutrients. No products exceeded federal safety limits for heavy metals, though 3 approached roughly half of the threshold of concern for lead. The findings echo a 2023 report from the US Government Accountability Office, which tested 12 best-selling prenatal supplements and found 11 had at least 1 nutrient deviating from its labeled amount, most frequently vitamin A and vitamin E.²

That report concluded that because dietary supplements are not subject to premarket approval, consumers have limited assurance that label claims reflect actual content.

Why Does Nutrient Accuracy Matter in Prenatal Formulations?

Pregnancy nutrient needs are narrower and less forgiving than general multivitamin use, and roughly 3 in 4 pregnant people in the United States use a dietary supplement during pregnancy.¹ Underdosing folate carries a defined public health consequence, while overdosing fat-soluble vitamins such as A and D in gummy matrices, which rely on different manufacturing processes than tablets, raises separate tolerability concerns.

How Are Manufacturers and Trade Groups Responding?

The Council for Responsible Nutrition, a trade association representing supplement manufacturers, pushed back on parts of the testing framework, noting that the USP tolerance ranges are voluntary for products not marketed as verified and that falling outside them does not equate to a violation of federal requirements.³

The association also pointed out that Consumer Reports' own conclusion found all 17 products safe, and that its results reflect a snapshot of 2 to 3 lots rather than ongoing production. Manufacturers, the association noted, are required to follow current good manufacturing practice regulations under federal law and are responsible for verifying that products are safe and properly labeled, with many investing internal quality systems, supplier qualification, and testing protocols beyond what regulation requires.3,

What Are the Limits of This Testing, and What Comes Next?

Both the Consumer Reports and federal findings are limited by small sample sizes relative to the thousands of prenatal products on the market, and neither tested for ingredient identity or bioavailability, only labeled potency against measured content. The Government Accountability Office recommended that Congress consider granting the FDA additional authority over supplement registration and labeling, a policy question that remains unresolved.

For manufacturers, independent verification through recognized certification programs remains a key tool to substantiate label claims.
